The State of Illinois requires various medical information for school children. The following forms are required:

|Preschool, Kindergarten and 8th grade||Vision Examination Report|
|Preschool, Kindergarten, 1st and 2nd||Hearing Examination Report|
|Preschool, Kindergarten,6th Grade||Up-To- Date Immunizations|
- Any middle school students who will be participating in a sport must receive a sports physical examination.
- All new students must have the following forms turned in before they begin school: Physical Examination, Up-To-Date Immunizations, Vision and Hearing Screenings.
- All students receiving special education services must have the Vision and Hearing Screenings completed.
Failure to provide the results of current physician’s examination with data about all required immunizations could result, according to state law, in exclusion from school until full details are reported.
