Health Immunizations and Screenings

All forms are due July 15, 2019 for the 2019-2020 school year. Failure to submit these forms on time will affect your child’s ability to attend school.

Required Forms

  • Forms to be filled out by healthcare professionals: 
    • Physical Exam Form You do not typically need to provide this form to the doctor—simply request it following a physical and submit a copy to the Main office
    • Dental Exam Form
    • Religious Immunization Exemption Form Ancona tracks which immunizations each student has to ensure that state of Illinois standards are met. If your child does not have all immunizations for religious reasons, you may submit this exemption form
  • All students are required to have an “Allergies and Conditions” form completed by their parent/guardian in My Ancona annually. This form is not to be confused with the “Physical Exam Form”, which is filled out by the child’s health care professional (see above) This information can also be found and updated at any time on the “Medical” tab of each student’s Contact Card — see instructions for updating Medical information
  • Students with allergies, asthma, diabetes, seizure disorder, or other medical conditions that require medications or accommodations at school will need an action plan form completed by health care provider.

P3 and P4

  • Physical exam including lead screen
  • Up-to-date immunizations (see below for specifics)
  • Vision screen**
  • Hearing screen**

Kindergarten

  • Physical exam including lead screen
  • Up-to-date immunizations (see below for specifics)
  • Vision screen**
  • Hearing screen**
  • Dental exam

1st Grade

  • Hearing screen**

2nd Grade

  • Vision screen**
  • Hearing screen**
  • Dental exam

3rd Grade

  • Hearing screen**

4th Grade

  • No requirements

5th Grade

  • Students who will be participating in a sport must receive a physical examination

6th Grade

  • Physical exam
  • Up-to-date immunizations (see below for specifics)
  • Dental exam
  • Students who will be participating in a sport must receive a physical examination

7th Grade

  • Up-to-date immunizations (see below for specifics)
  • Students who will be participating in a sport must receive a physical examination

8th Grade

  • Vision screen**
  • Up-to-date immunizations (see below for specifics)
  • Students who will be participating in a sport must receive a physical examination

New Students (all grades)

  • Physical exam
  • Up-to-date immunizations (see below for specifics)
  • Vision screen**
  • Hearing screen**

Preprimary 3

  • DTap (Age 2)
  • MMR
  • Polio (Age 1)
  • Varicella (Chicken Pox)
  • Hepatitis A (optional)
  • Hepatitis B (Age 2)
  • HiB
  • Invasive Pneumococcal Disease

Preprimary 4

  • All Pre-primary 3 immunizations
  • DTap (Age 4)
  • Polio (Age 4)
  • MMR Booster

Grade 6

  • All previous immunizations
  • TDap
  • Meningococcal Disease
