Click “Complete Now” next to your child’s name. Repeat for each student. The “To Do” marker will disappear once all requirements for your child are completed
You will first be presented with a series of “conditional questions” Answering “Yes” to a question will add a related item to your “To Do List” — don’t worry, you can change your answers later as needed
Complete each “To Do List” item — each item is self-explanatory.
Some items you submit must be approved by school staff, and will show “pending” just after you have submitted the form. Follow-up emails will be sent by the school as needed.
Please keep in mind that the forms collected in Magnus Health during the spring of 2021 are intended for use during the 2021-2022 school year, and the “Medical” area in My Ancona is still in use for the duration of the 2020-2021 year. Please reach out to if you need to make any changes to health information for the current school year. Once the transition is complete in fall 2021, you will be able to make adjustments directly within the Magnus Health system throughout the year.

For certain forms such as the Immunization Form (a.k.a Physical Exam Form) and Dental Exam, you’ll need to submit standard forms that are filled by your provider. The “Turn it in” button gives you options to upload the document directly or to download a special fax cover sheet that you or your provider can use.
