All forms for the 2018-2019 school year are due August 3, 2018

This page lists the screenings and immunizations required for each child based on their grade level. Please email all required to forms to gro.l1548284691oohcs1548284691anocn1548284691a@rar1548284691tsige1548284691r1548284691 or drop them off at the Main Office.

Hearing and vision screenings are conducted by a technician at Ancona during the fall of each school year. These screenings are available at no cost to families..

New Students 

Note that new students who are starting mid school-year must have all forms on file before their first day of school.

* Hearing and vision screenings are provided by Ancona in the Fall, so students starting in September do not need to undergo hearing and vision screenings before coming to Ancona.  However, new students starting mid school-year must be screened elsewhere before their first day of school.

Preprimary

Kindergarten

1st grade

  • hearing screen (provided by Ancona)

2nd grade

  • vision screen (provided by Ancona)
  • hearing screen (provided by Ancona)
  • dental exam

3rd grade

  • hearing screen (provided by Ancona)

5th grade

6th grade

7th grade

  • show proof of receipt of one dose of Tdap vaccine (containing tetanus, diphtheria, acellular pertussis). Most students may have already received the vaccine and simply need to provide the school with verifying documentation from the family health care provider.
  • students who will be participating in a sport must receive a sports physical examination

8th grade

  • vision screen (provided by Ancona)

Physical Exam Form — You do not typically need to provide this form to the doctor – simply request it following a physical and submit a copy to the front desk.

Dental Exam Form

Religious Immunization Exemption Form — Ancona tracks which immunizations each student has to ensure that state of Illinois standards are met. If your child does not have all immunizations for religious reasons, you may submit this exemption form.

Allergies and Conditions Form on My Ancona

My Ancona keeps allergy and health conditions on file in such as way that teachers and staff who give first aid have it easily accessible. It is a parent responsibility to ensure that this information is kept up to date. Each year in April, a required “Allergies and Conditions” form becomes available in My Ancona that allows you to view and update allergies, conditions, physician information, and medical insurance information. This form is not to be confused with the “Physical Exam Form”, which is filled out by the child’s health care professional (see above) This information can also be found and updated at any time on the “Medical” tab of each student’s Contact Card — see instructions for updating Medical information

Note that the paper “Medication form” is no longer used, as this information should instead be on file in My Ancona. For severe conditions, please fill out the appropriate action plan form found below and return them to the front desk:
